Tryout Request Form

Please include as much detailed information as possible for the Region to include in your event details on the website. i.e., Registration link, tryout fee, etc. 

Event Requirements & Guidelines

1. Clubs may set their own tryout fees and will be responsible for collection of all fees in addition to confirming that all players have a CURRENT USAV/ERVA membership.
    
2. Insurance certificate requests for your event must be requested no later than 7 days prior to the event. Proof of insurance is provided at no charge to the club.
    
3. All ADULT participants MUST have a current ERVA/USAV Membership, Current Background Screen and Current SafeSport & IMPACT certification.
