If you event is approved you will be emailed a Tournament Sanction Form.

1 FORM PER TOURNAMENT DATE i.e., U12 & U14 age groups on 1/6/2019= 1 form, U12 on 1/6/19 & U14 on 1/7/19= 2 forms
